Insurance Verification

Before your first session, please take a few minutes to confirm your mental health benefits so you know what to expect.

You can find the member services phone number on the back of your insurance card. When you call, let the representative know you’d like to verify coverage for outpatient mental health counseling using CPT code 90837 (individual therapy, 55 minutes).

Ask the representative the following:

1. Do I have coverage for outpatient mental health or behavioral health services?

2. Is pre-authorization or a referral required before starting therapy?

3. What is my copay, coinsurance, or deductible for each session?

4. Does my plan cover telehealth (virtual) sessions?

Be sure to write down the name of the representative, date of your call, and any reference number they provide for your records.

Please Note: It is the client’s responsibility to verify insurance coverage and understand any out-of-pocket costs before the first session. Clients are responsible for all fees not covered by insurance, including copays, deductibles, or denied claims.

If your plan offers out-of-network benefits, a superbill can be provided for possible reimbursement after payment.
